RCMP Criminal Background Check

All Coaches for Cloverdale Community Football Association are required to undergo an RCMP Criminal Background Check.

Failure to complete the process before the deadline [June 30th] will result in coaching privilidges being revoked  -- NO EXCEPTIONS

The process is quick and easy to complete.  You must complete the following steps:

 

  1. Print, complete and submit the Volunteer Disclosure Statement to the CCFA President
  2. Print and complete the RCMP Form 1 and RCMP Form 12
  3. Print and complete the CCFA Volunteer Letter.  You must have this letter if you wish the RCMP to waive the criminal check fee of $20.
  4. Take the completed RCMP Form 1 and RCMP Form 12 as well as the CCFA Volunteer Letter along with two pieces of identification (one must be a picture ID and one must contain your your current address to:  Surrey – RCMP

    Surrey BC V3X 1A9  (note only residents of Surrey register at the Surrey detachment, otherwise go to your local detachment).
  5. Keep the receipt you receive when you drop off your application. It is proof that you submitted your application
